Membership Agreement

I hereby apply for membership with Central Co-op understanding the conditions and policies stated in the introductory brochure and the Articles of Incorporation and Bylaws of the organization. I understand that a membership account is owned by the individual whose signature appears on the membership application. Additionally, I understand that domestic partners and household members are eligible to receive the benefit of my membership; however, the membership is not transferable. As well, if I should die or if my membership account remains inactive for five (5) years. I understand that my equity investment will be transferred to Central Co-op and that I have three (3) years in which to reinstate my membership or forfeit all rights of membership. I can resign my membership at any time and a refund of my membership equity will be mailed to me within sixty (60) days.
